Please make an appointment to meet Miss Jan, tour the facility, and fill out these forms.  If you decide to enroll:

Immunizations: Shot records need to be provided before care can begin.

Health clearance:  Your doctor fills out the Health clearance form.  This can be turned in within two weeks of your child's first day of attendance.

Contact form: List all people that can be contacted in case of emergency and names of people who may pick up your child. 

Contract: This sets in writing responsibilities of both the provider and parents. This includes both full-time, part-time, and drop in care.

Child's Interests:  This provides information about your child. This can be anything you want to share about your child that will help the provider understand your child.
